Panel Coordinator (Temporary) 0.8 FTE
Description:
The Panel Coordinator (PC) will work with the medical home’s electronic medical record (EMR) to identify and contact patients due for preventative health screening (e.g., those due for screening such as mammograms) and care appointments. This may involve performing chart reviews or running reports on the EMR.
This position will work with multiple PCN Medical Homes on a rotating basis. The Panel Coordinator (PC) works with physicians, medical home staff, and clinical team members. A combination of in-person/medical home, central site and remote work may be required.

QUALIFICATIONS:
Education and Certifications
- Medical Office Assistant/Unit Clerk certification preferred
- Medical terminology course
- Combination of equivalent education and experience may be considered
- Valid driver’s license and access to vehicle
Experience
- Minimum of one year previous experience in primary care setting
- Knowledge of Electronic Medical Record system and NetCare an asset
Skills and Attributes
- Excellent oral and written communication skills
- Ability to balance multiple priorities and use problem-solving skills
- Ability to work independently
- Excellent computer knowledge, including M365 applications
- Minimum of one year previous experience in primary care setting
